GDPR Photographic Consent-

For use of Photographs, video and digital images. By accepting below I give permission to take photographs and/or video of my child. I grant Creationz Dance Academy full rights to the use of the images resulting from the photography/ video filming, and any reproductions or adaptions of the images for fundraising, publicity or other purposes to help achieve the Academy's aims. This might include (but is not limited to), the right to use them in printed and online publicity, Social Media, Press releases and funding applications.

Rules:

• Hair Must be tied back out of students face.
• No jeans, please wear legging or comfortable clothing that allows us to see your lines.
• No chewing Gum
• Bare foot or Jazz shoes only.
• Parents are to remain outside of class for the duration of the lessons (parents will only be allowed to watch in exceptional circumstances at Miss Zoe's discretion)
• Uniform T-shirt is needed within 4 weeks of starting their first lesson, we want you all to look neat!
• No phones allowed in lessons.
• We are zero tolerance towards any bullying, we are supportive community!
• Missed lessons must be paid for. Missing classes can impact your progress.
• No Smoking or Vaping on grounds of venue.
• Parents are to collect and drop their child off inside the dance venue. Children are not our responsibility before and after the allocated class time.
• Any Child wishing to Participate in Creationz Tribe Performance Group must also attend a “main class”.

By agreeing to All of the above statements you are acknowledging that you have read and agree to abide by the rules.
